is a simple step by step process: Step 1 -
You will need the following.
- Printer with Letter Size Paper (8.5 x 11)
- You will need free "Adobe Acrobat Reader Software" to view
.pdf files. (if you do not have Adobe Acrobate you can download by double clicking the
version you need:

CalPERS Mortgage
Loan Certification
Step 3 - Save forms to your computer, double click
and print.
Step 4 - Complete form, and mail
or fax to our office attention: CalPERS Home Loan Department.
